Job Duties

Patient Care

• Work with geriatric patients
• Complete Medicare annual health assessments during home visits

Patient Load

• 5-6 patients/day
• This role includes seeing patients in home visits

Clinical Responsibilities

• Perform H&Ps
• Use preventative health screening tools (PAD and Neuropathy, diabetic retinal cameras, A1c and kidney testing for diabetics)
• Navigate and chart via EMR systems
• This position focuses on completing comprehensive health assessments and collaborating with patients' primary care providers to ensure continuity of care. You will enhance patient outcomes through detailed evaluations and effective communication, but this is not a primary care or PCP role.

Training

• We provide comprehensive training to ensure you feel confident and prepared in your role. Our supportive environment means you won't be left to navigate challenges alone.
• All equipment provided including laptops and tablets.

Requirements

• Current NP license for the state
• Ability to use a computer efficiently
• EMR systems and electronic documentation
• Professional, positive, passionate, and flexible
