PharmaCord is urgently hiring a Remote Bilingual (Spanish/English) Registered Nurse Case Manager to support our growing patient engagement and care coordination services. Job Description:

The RN Case Manager will provide personalized, telephonic care coordination for patients on specialty medications. You’ll be the empathetic voice patients turn to for medication education, follow-up support, and help navigating insurance or healthcare barriers. Bilingual fluency in Spanish and English is required to serve our diverse patient population effectively.

Responsibilities:

  • Conduct outreach and care coordination calls in both Spanish and English

  • Educate patients on prescribed therapies and monitor medication adherence

  • Coordinate with physicians, pharmacies, and payers to resolve care or access issues

  • Document case notes and track outcomes in compliance with HIPAA regulations

  • Build trust-based relationships with patients and caregivers over time

  • Provide culturally sensitive, respectful, and inclusive care support

Requirements:

  • Active RN license in the state of Illinois (multistate license preferred)

  • Bilingual in Spanish and English (verbal and written fluency required)

  • Minimum of 2 years of nursing experience, preferably in case management or chronic care

  • Strong communication, documentation, and organizational skills

  • Tech-savvy with experience using EMRs, CRM systems, or patient portals

  • Must have a dedicated, private home workspace and reliable internet
