Job Description We're looking for a skilled freelancer to design a professional vendor form similar to the attached sample from. This form will be used by blithequark to onboard customers (hospitals, surgery centers, doctor offices, schools, etc.) and ensure a seamless onboarding experience. The ideal candidate will have proficiency in LaTeX, Adobe Acrobat, or similar tools for form design, experience creating business forms or templates, and a keen understanding of U.S. business and tax requirements.
