Job Summary

We're seeking a highly skilled and empathetic Licensed Social Worker to join our team as a Remote Medical Case Manager and Advocate. In this role, you'll work closely with a registered nurse case manager to assess, plan, and advocate for the needs of our members. Your expertise and compassion will be essential in empowering our members to make informed decisions about their health and well-being, and in connecting them with vital resources and support services.

Responsibilities

As a Remote Medical Case Manager and Advocate, your key responsibilities will include:

  • Conducting thorough assessments of members' needs and developing personalized plans to address their health and wellness goals
  • Collaborating with registered nurse case managers to provide comprehensive care coordination and advocacy services
  • Researching and connecting members with local resources, services, and support systems to address financial, transportation, safety, and housing needs
  • Empowering members to make informed decisions about their health and well-being through education, support, and advocacy
  • Working closely with other clinical staff to ensure seamless care coordination and effective communication with members and their families
  • Maintaining accurate and up-to-date records of member interactions, assessments, and care plans

Requirements

To be successful in this role, you'll need:

  • An active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in your state of residence
  • At least 2 years of experience in medical social work or case management
  • A Master's degree in Social Work or a related field
  • Excellent communication, interpersonal, and problem-solving skills
  • Ability to work independently and as part of a team
  • Strong analytical and organizational skills
  • Proficiency in electronic medical records and other healthcare software systems
