RN - Quality Improvement Specialist PRN

Empath Home Health, a member of Empath Health, is seeking a Registered Nurse RN Quality Improvement Specialist PRN.

The Quality Improvement Specialist is a unique position responsible for monitoring internal and external quality measures across the agency. The Quality Improvement Specialist proactively participates in the development and implementation of the organization’s strategic initiatives, performance improvements, and creates value for both internal and external customers. Responsible for overseeing quality of customer care including customer experience, safety and analyzing key quality indicators to identify areas for improvement. and meet regulatory and quality standards.

Schedule: PRN; must be able to commit to at least 20 hours per week (to be worked during normal business hours)

Location: remote

Requirements:

  • Education and/or Experience: Registered Nurse or healthcare professional with minimum of associate degree in related field, bachelor’s degree preferred. 
  • CPHQ preferred or equivalent
  • Experience monitoring healthcare external and internal patient satisfaction, quality measures, and outcome measures. Hospice, Home Health or Palliative Care standards and quality measures.
  • Exceptional critical thinking skills enhanced with the finesse to prioritize key aspects and communicate findings with diverse audiences.
  • Establish standards to facilitate continuous improvement. Facilitate project meetings focusing on goals, interventions, and timelines.
  • Knowledge of quality improvement methods and models (e.g. Rapid Cycle, Lean Six Sigma, Baldrige, PDSA)
  • Proficient in Microsoft Office Applications including Excel, Word, and PowerPoint to create reports.
  • Experience in customer service. Strong customer advocate committed to the continual improvement of quality, safety, and service excellence.
