We're now seeking an experienced Case Management Specialist to join our remote customer service team, working from the comfort of your own home.

About the Role

As a Case Management Specialist, you'll play a vital role in helping us achieve our mission of providing exceptional care and support to our members. You'll work closely with our team to assess the needs of our members, develop and implement care plans, and provide education and support to help them achieve their health and wellness goals. This is a full-time remote position, working Monday through Friday, 8:00 am to 5:00 pm CST.

Program Overview

Our Double Qualified Extraordinary Requirements Plan (DSNP) is a comprehensive program designed to provide support and resources to our members who are enrolled in Federal Medicare and Medicaid and face complex health and social challenges. As a Case Management Specialist, you'll work with our team to develop and implement care plans that address the unique needs of our DSNP members, helping them to achieve optimal health and wellness outcomes.

Key Responsibilities

* Conduct comprehensive assessments of members' needs and develop care plans to address their health and social challenges

  • Collaborate with healthcare providers, community organizations, and other stakeholders to ensure seamless care coordination and support
  • Provide education and support to members to help them make informed decisions about their health and wellness
  • Identify and address high-risk factors and service needs that may impact member outcomes
  • Develop and implement quality improvement initiatives to enhance the quality of care and services provided to our members
  • Utilize case management and quality management processes to ensure compliance with regulatory requirements and company policies

Essential Qualifications

* 3+ years of experience in a health-related field, preferably in case management or a related field

  • Bachelor's degree or equivalent experience required
  • Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders
  • Ability to work in a fast-paced, dynamic environment, with a strong focus on customer service and member satisfaction

Preferred Qualifications

* Certification in case management (CRC, CDMS, CRRN, COHN, or CCM)

  • Experience working with geriatric populations, social health, and disability populations
  • Bilingual (English/Spanish; English/Creole) or multilingual skills
  • Knowledge of evaluation, screenings, and care planning
  • Experience working with Federal health insurance and Medicaid programs

* Competitive salary range: $19.52 - $40.90 per hour
