[Hiring] CCM Intake Medical Assistant @Nsight Health
Role Description We are seeking a motivated and detail-oriented Medical Assistant to join our Chronic Care Management team. In this role, you will play a critical role in patient care by guiding patients through health assessments, reviewing medications and diagnoses, troubleshooting medical devices, and escalating clinical needs to nursing staff when appropriate.
Key Responsibilities
- Support CCM nursing staff by completing monthly patient assessments and conducting Chronic Care Planning Assessment (CCPA) calls, ensuring a positive, informative, and patient-centered experience.
- Complete delegated clerical and administrative activities on behalf of nursing staff to support patient engagement, improve care coordination, and drive quality outcomes and overall patient experience.
- Document patient interactions, assessments, and intake in the EHR.
- Maintain compliance with company policies and applicable regulations.
- Coordinate communication between patients, caregivers, nursing staff, and providers to ensure continuity of care and timely follow-up on patient needs.
- Perform other duties, as assigned.
Qualifications
- Active MA license required.
- Strong communication and organizational skills.
- Bilingual Proficiency: Fluent in English and Spanish.
- Preferred: Passionate about patient care, possessing the ability to relate with empathy and compassion. A passion for patient teaching is a must.
Requirements
- Minimum fiber internet speed of 50 Mbps download / 10 Mbps upload.
- Hardwired internet connection required.
- Speed test submission required during the offer process.
- Private, HIPAA-compliant workspace.
Schedule
- Monday - Friday 8:00 am - 5:00 pm EST
- Monday - Friday 9:00 am - 6:00 pm EST
- Monday - Friday 10:00 am - 7:00 pm EST
- Monday - Friday 11:00 am - 8:00 pm EST
Training Requirements All new hires must complete a comprehensive training program:
- Duration: Five weeks
- Schedule: Monday through Friday, 9:00 AM – 6:00 PM Eastern Time
- Attendance is mandatory to ensure readiness prior to independently supporting patients.
Compensation & Benefits
- Competitive base pay: $18-$20/hr
- Shift Differentials:
- Evening Differential: +$1.50/hour for hours worked after 7:00 PM ET
- Late-Night Differential: +$2.00/hour for hours worked after 10:00 PM ET
- Weekend Differential: +$1.50/hour for all hours worked Saturday and Sunday
- Additional Compensation (If Applicable):
- Sign-On Bonus: $2,000 paid after 120 days of continuous employment, contingent upon active employment and satisfactory performance at time of payout.
- Monthly Bonus Potential: Up to $1,000
- 5% Bilingual Pay Allowance
