Job Summary

We're seeking a highly skilled Full Stack Social Worker to join our remote medical case management team. As a key member of our team, you'll work collaboratively with a registered nurse case manager to assess, plan, and advocate for our members' benefit plans and health needs. Your expertise will be instrumental in researching available options and resources for members with financial, transportation, safety, and housing needs. This is a remote work-from-home role, and you'll have the flexibility to work from anywhere in the U.S.

Responsibilities

* Collaborate with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs.

• Research available options and resources for members with financial, transportation, safety, and housing needs.
• Empower members to make positive lifestyle and medical choices.
• Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health.
• Conduct telephonic or electronic exchanges with members to assess their needs and develop personalized plans.
• Develop and maintain relationships with community resources and organizations to ensure seamless access to care.

Requirements

* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence.

• 2+ years of experience in medical social work or case management.
• Masters Degree in Social Work.

• Strong communication and interpersonal skills, with the ability to build rapport with members and community resources.
• Ability to work independently and as part of a team.
• Strong problem-solving and critical thinking skills.

• A base salary range of $46,597.20 - $101,228.40.
