Job Summary

We're seeking an experienced Case Management Analyst to join our remote team, working on a part-time basis. As a Case Management Analyst, you'll be responsible for assessing client needs, developing and implementing care plans, and ensuring that our clients receive the highest quality care and services. You'll work closely with our team of healthcare professionals, clients, and stakeholders to drive business growth and success.

Key Responsibilities

* Conduct comprehensive assessments of client needs and develop individualized care plans to meet their goals and objectives

  • Collaborate with healthcare professionals, clients, and stakeholders to ensure that care plans are implemented and monitored effectively
  • Identify high-risk factors and service needs that may impact client outcomes and develop strategies to address these issues
  • Facilitate and coordinate care plan activities and monitor progress to ensure that clients receive the highest quality care and services
  • Develop and maintain relationships with clients, healthcare professionals, and stakeholders to ensure that care plans are implemented and monitored effectively
  • Use case management and quality assurance processes to ensure compliance with regulatory and accreditation standards
  • Participate in multidisciplinary case conferences to develop and implement care plans that meet client needs and goals
  • Use data and analytics to identify trends and opportunities for improvement in care delivery and business growth

Essential Qualifications

* Bachelor's degree in a related field, such as social work, psychology, or healthcare administration

  • 2+ years of experience in case management or a related field
  • Strong knowledge of case management principles, practices, and regulations
  • Excellent communication, interpersonal, and problem-solving skills
  • Ability to work independently and as part of a team
  • Strong analytical and problem-solving skills
  • Proficiency in Microsoft Office applications, including Word, Excel, and PowerPoint

Preferred Qualifications

* Master's degree in a related field, such as social work, psychology, or healthcare administration

  • Experience in case management and release planning
  • Supervisory experience in a healthcare setting
  • Certification in case management or a related field

Skills and Competencies

* Strong analytical and problem-solving skills

  • Excellent communication, interpersonal, and leadership skills
  • Ability to work independently and as part of a team
  • Strong knowledge of case management principles, practices, and regulations
  • Proficiency in Microsoft Office applications, including Word, Excel, and PowerPoint
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities
  • Strong attention to detail and organizational skills
