PRN Registered Nurse - Infusion Support
Job Type Part-time Description TwelveStone Infusion Support manages the Infusion needs of patient's and maintains the continuum of care, from intake, assessment, and treatment through the completion of their visit. Our primary focus is the safe and effective administration of biologic medications (e.g., Remicade, Tysabri, etc.). We are currently hiring for the position of PRN, Registered Nurse - Infusion Support in Virginia. This position is responsible for managing the individualized patient care by promoting and restoring patients' health through the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, and families; and providing comprehensive clinical care, including the assessment, treatment, management and education for patients with acute or chronic health needs in the home setting. You will be the RN responsible for safely and efficiently delivering infusion therapy to our customers suffering from chronic diseases and acute medical illnesses. We welcome RNs from all medical backgrounds with a positive attitude, preferably possessing outstanding IV skills, and requiring a license in the state of practice. Responsibilities include: Performs analysis of patient data including lab work, patient assessments; reports changes to referring physician; makes recommendations for patient care; performs initial and ongoing patient assessments and provides patient education. Conducts individualized patient assessments, prioritizing the data based on the patient's immediate condition or needs within timeframe specified by company policies, procedures, or protocols. Facilitates consistent and clear communication among all members of the healthcare team. Incorporates the patient and family in care planning and treatment. Administers medications, infusion therapy, and other appropriate treatments per prescriber orders. Provides patient-centered infusion care. Serves as an advocate for patients in navigating the healthcare system. Obtains referrals from intake department to initiate home health visit. Schedules visit time with patients/caregivers and confirms appointment in EMR. Documents, verifies, and ensures implementation of physician orders. Documents accurate and timely patient assessment findings, physical/psychosocial responses to nursing intervention and progress towards goals of infusion therapy. Delivers care with appropriate age and cultural competence according to individualized needs in a non-judgmental, non-discriminatory manner that is sensitive to patient's diversity while preserving their autonomy, dignity, and rights. Maintains confidentiality in matters related to patient, family, and company staff. Actively participates in quality initiatives regarding patient services and program reviews.
Requirements
Minimum Qualifications: Education: Bachelor's or Associate degree in Nursing; current, active and unrestricted RN license in the state of practice and current CPR certification (BLS minimum) required. Experience: Minimum of one year experience providing nursing care required. Infusion experience required. Telephone, MS Office and computer/keyboard skills required. Previous EMR experience preferred. Must be knowledgeable of operations processes and guidelines for communication and PHI.
