Centralized Admissions Coordinator - MN (Remote)

Position: Centralized Admissions Coordinator - MN (Remote) As the Centralized Admissions Coordinator, you will play a pivotal role in ensuring the seamless and efficient admission process for our communities. You will be responsible for a range of tasks that contribute to the success of our organization and the satisfaction of our residents and their families. Responsibilities:

• Referral Review: Thoroughly and promptly review all incoming referrals, ensuring accuracy and completeness in clinical and financial documentation.
• Background Checks: Conduct and review criminal background checks on potential residents, adhering to all applicable regulations and policies.
• Financial Analysis: Verify and evaluate financial information, including primary insurances and Medicaid eligibility, to determine the financial viability of admissions.
• CRM Management: Maintain and update the Customer Relationship Management (CRM) system with lead information and manage leads effectively.
• Prior Authorizations: Initiate and oversee the prior authorization process when necessary, working closely with healthcare providers and payers.
• Document Management: Upload all admission-related documents into the designated system and ensure timely notification to the accepting facility.
• Provide direction to liaison/s for on-site or additional information needed
• Ad Hoc Duties: Perform additional tasks as assigned by the management to support the smooth functioning of the admissions process.

Requirements:

• Must have prior admissions experience in a Skilled Nursing Facility, preferably in Minnesota.
• Should be residing in Minnesota
• A high school diploma or equivalent; bachelor's degree in a related field is a plus.
• 3 years experiences in skilled nursing admissions is required
• Proficiency in utilizing computer systems (Point Click Care) and CRM software.
• Excellent organizational and communication skills.
• Ability to work independently and collaboratively within a team.
• Familiarity with healthcare regulations and insurance processes is advantageous.

Job Type: Full-time Schedule:

• Monday - Friday
• 8:00 AM - 4:30 PM
• No weekends/holidays

Experience:
