Caregiver Coordinator (flexible hours/location/work from home)

Summary: The Care Coordinator/Scheduler will focus on meeting Client needs and requirements and maintaining Client relationships by identifying the best caregiver/client match, scheduling care, and ongoing case management. Essential Skills/Qualities: • Team player with a positive attitude - dedicated to the mission! • Nurturing ability (caring, empathetic, compassionate), high level of emotional intelligence and empathy • Articulate, with strong verbal and written skills • Creative problem-solving skills, organized and detail oriented, strong documentation skills. • Flexible and able to multi-task, effective time management skills • Proactive - willing to take initiative and follow through • Capable of performing job functions free from immediate direction or supervision • Excellent phone skills/etiquette • Excellent computer skills, proficient with Microsoft Office and browser-based applications • Able to effectively work from home in a productive and professional manner • Must have reliable internet connectivity, be able to use standard business equipment including a desktop/laptop computer and a mobile phone as well as all the associated apps and software • Works closely with the Caregiver Coordinators and Case Managers to create a complete, successful, satisfying, and on-going life cycle of exceptional services to our clients and Caregivers Primary Duties working with Clients: • Fills cases with existing, qualified Caregivers by using empathy to make the best possible match. • Assign or refer appropriate caregivers to clients, based upon the specific needs and requirements of each client and the qualifications and experience of the Caregiver • Coordinate scheduling activities, including assignments, replacements and substitutions of Caregivers • Interact with Caregivers and clients and take any action necessary to ensure that the specific needs and requirements of each client are met • Resolve any client, family, and Caregiver differences or disputes on behalf of the Company • Exercise discretion and independent judgment in the performance of each of the foregoing activities Primary Duties working with Caregivers: • Keeps notes and files throughout the recruiting process, both in ClearCare and other necessary systems as well as physical paper files for Caregiver’s employee file folder. • Takes selected Caregivers through the on-boarding process and creates the comprehensive Caregiver’s file, adding important dates regarding expirations into the Caregiver’s profile in ClearCare and Payroll systems (possible expirations include medical clearances, driver’s licenses, car insurance (if necessary), government issued photo ID, and criminal background check. • Performs regular maintenance of the Caregiver’s files doing quarterly audits and obtaining updated information and documents from Caregivers to ensure that all records are up to date • Maintains regular contact, usually via phone, with Caregivers to check in and keep the Caregiver connected to the Company. • Acts in accordance with applicable Federal and State laws governing interviewing and on-boarding. • Utilizes Outlook for email and calendar appointment/event scheduling. • Utilizes ClearCare and any other necessary systems for complete and accurate caregiver records including notes and follow-up. • Must be able to interpret information in our systems (e.g., ClearCare, Email, etc.) and accurately exchange information with others, as well as record information in these same systems.
