Job Overview As an LPN, you will provide direct patient care and work alongside a diverse team of healthcare professionals, ensuring a safe and nurturing environment for our residents. Your expertise and compassion will be instrumental in shaping the care experience. Key Responsibilities Deliver direct patient care, including the administration of medications and monitoring of vital signs. Create and implement personalized care plans tailored to each resident's needs. Collaborate effectively with a multidisciplinary team, including physicians, nurses, and therapists. Uphold resident safety and comfort by adhering to infection control protocols and maintaining a hygienic environment. Educate residents and their families on health-related topics, fostering an understanding of care processes. Maintain accurate and timely documentation in resident medical records for continuity of care. Required Skills Current and unencumbered LPN license in the State of South Dakota. Excellent communication and interpersonal skills. Empathetic and patient-centered approach to care. Strong team player with the ability to work collaboratively. Qualifications Completion of an accredited LPN program. Experience in long-term care or a similar healthcare setting is preferred but not mandatory.
