Remote RN Care Coordinator (Part-Time to Potential Full-Time)
We are seeking a dedicated and experienced Registered Nurse (RN) Care Coordinator for a fully virtual position supporting population health and care coordination efforts. Candidates must hold an active RN license in New York, Texas, or Oregon. This role will begin as part-time with the potential to transition into a full-time opportunity based on performance and program needs.
The ideal candidate will have prior experience working in an Accountable Care Organization (ACO) or similar value-based care environment. A background in acute care nursing and proficiency with Salesforce or other care management platforms is highly desirable.
Key Responsibilities:
- Provide remote care coordination for a panel of patients in alignment with ACO goals
- Conduct telephonic outreach, health assessments, and care planning
- Collaborate with interdisciplinary teams to ensure appropriate care transitions and follow-up
- Identify and address gaps in care, social determinants of health, and patient barriers
- Document all interactions and interventions in Salesforce or designated system
- Monitor outcomes and escalate complex issues to clinical leadership as needed
Requirements:
- Active RN license in New York, Texas, or Oregon (required)
- Minimum 3 years of clinical nursing experience, with a preference for acute care
- Prior experience in ACO, care management, or population health
- Comfortable with technology and remote work platforms
- Strong organizational and communication skills
Preferred Qualifications:
- Salesforce or care management platform experience
- Experience with virtual/telehealth care coordination
- Bilingual skills a plus
