About the position
The Registered Nurse - RN - Case Management position at CVS Health is a full-time, remote role focused on engaging and advocating for vulnerable members with complex medical, behavioral, and social challenges. The role involves assessing, planning, implementing, and coordinating case management activities to enhance members' overall wellness and health outcomes. The RN will utilize clinical tools, collaborate with interdisciplinary teams, and apply motivational interviewing skills to ensure effective member engagement and care planning.
Responsibilities
- Assess, plan, implement, and coordinate all case management activities with members. ,
- Evaluate the medical needs of members and facilitate their overall wellness. ,
- Develop proactive courses of action to enhance short and long-term outcomes for members. ,
- Conduct evaluations of members' needs using clinical tools and data review. ,
- Incorporate strategies to reduce risk factors and address complex health and social indicators. ,
- Conduct assessments considering various sources of information for co-morbid and multiple diagnoses. ,
- Use a holistic approach to assess the need for referrals to clinical resources and interdisciplinary team members. ,
- Collaborate with supervisors and key stakeholders to overcome barriers in meeting healthcare goals. ,
- Utilize case management processes in compliance with regulatory and company policies. ,
- Employ motivational interviewing skills to engage members and assess their health needs.
Requirements
- Registered Nurse with an unrestricted nursing license and multi-state/compact privileges. ,
- 3+ years of clinical/nursing practice experience. ,
- 1+ year(s) of medical case management experience, care coordination, or discharge planning. ,
- 3+ years of experience with personal computers, keyboarding, multi-systems navigation, and MS Office Suite applications. ,
- COVID vaccination is required.
