This position is based in Marathon Florida and requires frequent travel within the Lower Keys. Orientation, Onboarding & Training You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an R.N is 7-9 weeks, however, is individualized according to your needs and experience. Technology Use Hospice RN Case Managers utilize and leverage: HCHB = Proprietary Real-time database Optum Pharmacy solutions with 24/7 access IPrescribe - efficiencies with medication management Forcura - Document management system Medline - supply ordering system TalentEDGE - Amedisys' premier learning center EBSCO - Nursing Resource Center for CEUs Tablet with voice dictation and mileage tracking Outlook added to Teams, Chat, Yammer Responsibilities Performs initial nursing assessment on new admissions. Completes, coordinates and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff. Regularly communicates patient progress to the clinical manager, physician and care team. Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies. Records pain/symptom management changes/outcomes as appropriate. Educates families on patient condition, needs and care. Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so. Performs on-call responsibilities and on-call services to patients/families as assigned. Performs other duties as assigned.

Qualifications

Current RN license, specific to the state(s) you are assigned to work. One+ year of nursing experience in acute care setting(s) required. Current CPR certification. Valid driver's license, reliable transportation, and liability insurance.
